Team Registration

Please fill out the following registration form to register your team for the Powerplay Cup tournament!

You should receive a confirmation e-mail once you have submitted your registration. Closer to the tournament date, you will be contacted to attend a mandatory Captain’s meeting.

After you have received your registration confirmation e-mail you have two weeks turn in your tournament fee or your spot will not be secured in the tournament. The tournament fee is $250/team and guarantees 3 games as well as a tournament t-shirt/player.

On the day of the event you will be required to fill out a waiver. If you or a team member is under 18, please have a parent or guardian available to sign the waiver. Waivers will be given out at the start of the tournament, but you are also welcome to have one prepared before-hand. For the waiver please click here.
